100WomenStrong is excited to offer another grant stipend opportunity to nonprofit partners interested in joining the UNITE US/Unite Virginia Coordinated Care Network for easy referrals for clients.
Exciting data from our first Stipend Program for Loudoun Nonprofits shows:
More people are being connected to care through the Unite Us Platform.
- Referral volume in 2024 was up 148% compared to 2023 (700 total referrals impacting 363 clients)
Providers are finding the right-fit program more often.
- Referral acceptance rate increased from 40% in 2023 to 65% in 2024 (state acceptance rate goal = 65%)
Partners are communicating more efficiently.
- 80% of Unite Us referrals made in Loudoun County are acted on within 4 days (59% in 2023)
The network of social care providers is growing.
- As of 12/31/2024, there are 102 active organizations in the Unite VA Network (Active = sent or taken action on a referral within the past 3 months).
Key partners, such as Loudoun County Public Schools and HealthWorks, have joined or re-engaged with the platform since the launch of the cohort 1 organizations. Many cite the 100WS pilot program as the impetus to move forward with engagement.
Human services nonprofits are connecting around community priorities.
Since launch of the original organizations, Loudoun County human services nonprofits have established an ongoing Communities of Practice meeting to connect and strategize around community priorities. This is the first and only CoP meeting in Northern Virginia at this time and is paving the way for long-term network growth!
Additionally, Unite Us is exploring this onboarding process as a best practice to be used in other region and state networks.
